How to Schedule a New Patient Consultation

    1. Click the “Patient Portal” link in the menu bar above.

    2. On the lower right hand side of the new webpage that opens up, click “Create an account to book an appointment.”

    3. Enter your email address and proceed through the email verification process.

    1. After verifying your email address, you will be asked to fill out a form with basic personal information.

    2. At the end of this form you will be prompted to create a password. Once you’ve done this, you officially have a patient portal account and will receive a welcome email.

    1. In the upper center of the screen there will be a link that reads “Complete Forms”. You can click on that to navigate to the forms section or click on the “Forms” icon to the left hand side of the screen.

    2. Please thoroughly fill out each form. You will see them listed to the left side of the screen and each one will receive a check mark beside it once it has been completed.
